You can add it in the 'Apps' area when adding a section or a block in the Shopify theme editor. You can find more detailed instructions in the Publish area for your quiz, as shown below.
What if I don't see the option to add the quiz?
If you don't see the ability to add the quiz in a section or block, then it's likely that you're using an older Shopify 1.0 theme. If you are, we recommend upgrading to a modern 2.0 theme to take advantage of the latest Shopify features.
However, if you don't upgrade to an Online Store 2.0 theme, you can still add our quizzes to your site using the Embed code snippet instructions in the Publish area for your quiz, as shown below.
